Our team will assist our community of patients/residents and facilities in maintaining function of long term patients/residents, rehabilitation of those recovering from acute illnesses, and to preserve the dignity of those in the last stages of life. All of the care these members of our community receive, no matter their need and stage of life, will be a cooperative effort among the care providing team.
